ABSTRACT

Objective:To explore the expression and predictive value of serum CD36 level in cardio-vascular risk in patients with rheumatoid arthritis (RA) and to provide a new theoretical basis for clinical iden-tification and prediction of patients at moderate and high cardiovascular risk in RA patients.Methods:Eighty-four RA patients hospitalized to the Department of Rheumatology and Immunology of the second hospital of Lanzhou University in Gansu Province were selected as the study subjects, and 34 healthy people were selected as the controls. All RA patients were divided into low cardiovascular risk groups and medium and high cardiovascular risk groups according to prediction for ASCVD risk in China (China-PAR) cardiovascular risk assessment. At the same time, they were tested for serological indicators at admission. Statistical Product and Service Solutions (SPSS)) 26.0 software was used for the statistical data analysis. T-test and nonparametric test were used for comparison of the measurement data. The Chi-square test and Fisher's exact test were used for the comparison of classified data. Variables with P<0.05 in univariate analysis were included into the Logistic regression model. Results:Compared with the healthy control group, the number of patients with medium and high cardiovascular risk in RA[8 cases (23.5)% and 38 cases (45.2)%, χ2=4.80, P=0.029] was significantly higher and was negatively correlated with serum CD36 ( r=-0.27, P<0.05). Logistic regression analysis showed that age [ OR(95% CI)=1.654(1.157, 2.365), P=0.006] and diastolic blood pressure [ OR(95% CI)=1.225(1.040, 1.442) , P=0.015] were independent risk factors for medium and high cardiovascular risk in RA patients, and serum CD36 level [ OR(95% CI)=0.569(0.352, 0.922) , P=0.022] was the protective factor of medium and high cardiovascular risk in RA patients according to the results of Logistic regression analysis. The area under the receiver operating characteristic (ROC) area under the curve (AUC) was 0.691, the cut-off value was 4.27 pg/ml, and the sensitivity and specificity were 89.7% and 41.0% respectively. Conclusion:The serum CD36 level decreases with the increase of cardiovascular risk in RA patients. A higher serum CD36 level is a protective factor of cardiovascular risk in RA patients, and CD36 has a certain predictive value for cardiovascular risk in RA patients and is a potential predictor.

ABSTRACT

OBJECTIVE:To observe the efficacy and safet y of roxadustat in the treatment of chronic kidney disease (CKD) combined with renal anemia. METHODS :The data of 80 inpatients with CKD combined with renal anemia treated in the nephrology department of the First Affiliated Hospital of Anhui University of Science and Technology during Nov. 2019 to Dec. 2020 were analyzed retrospectively. They were divided into observation group (40 case)and control group (40 case)according to different treatment methods. Control group was given Human erythropoieti n for injection subcutaneously ,100-150 IU/kg every week,3 times a week. Observation group were given Roxadu- stat capsules orally ,100 mg each time for dialysis patients with a bodyweight of 45-<60 kg,70 mg each time for non- 717060193@qq.com dialysis patients ,3 times a week ;120 mg each time for dialysis patients with bodyweight ≥60 kg and 100 mg each time for non-dialysis patients ,3 times a week. After 4 weeks of treatment ,the dosage of the two groups w as adjusted according to the level of hemoglobin (Hb). Treatment course of 2 groups were 12 weeks. The levels of anemia indexes (Hb,red blood cell count (RBC),hematocrit(Hct)),iron metabolism indexes (serum iron (Fe),ferritin(Fer),total iron-binding capacity (TIBC),tranfeerrin saturation (TSAT)),lipid metabolism indexes (total cholesterol (TC),triglycerides(TG),high-density lipoprotein (HDL),low-density lipoprotein (LDL))and inflammation index(C-reactive protein (CRP))were detected in 2 groups before treatment and after 12 weeks of treatment. The levels of Hb were observed in high-level CRP subgroup and low-level CRP subgroup ;the occurrence of ADR was recorded in 2 groups during treatment. RESULTS :Before treatment ,there was no statistical significance in the anemia indexes ,iron metabolism indexes ,lipid metabolism indexes or inflammation index between 2 groups(P>0.05). After treatment ,the levels of Hb ,RBC and Hct in 2 groups and the levels of TIBC and HDL in observation group were significantly higher than before treatment. The levels of Fe ,Fer and TSAT in 2 groups and the levels of TC and LDL in observation group were significantly lower than before treatment ;the levels of Hb ,RBC,Hct,TIBC and HDL in observation group were significantly higher than before control group ,while the level of TC was significantly lower than control group (P<0.05). The incidence of high reactivity in observation group was significantly higher than control group ,while the incidence of low reactivity was significantly lower than control group (P<0.05). There was no statistical significance in the levels of CRP between 2 groups before and after treatment (P>0.05). Subgroup comparison results showed that there was no statistical significance in the level of Hb between high-level CRP subgroup and low-level CRP subgroup of 2 groups before treatment (P>0.05). After treatment ,the level of Hb in high-level CRP subgroup of observation group was significantly higher than that of high-level CRP subgroup of control group (P<0.05). The total incidence of ADR in observation group was significantly lower than control group (P<0.05),and no severe ADR was observed in 2 groups. CONCLUSIONS : Roxadustat can effectively improve anemia in CKD patients with renal anemia ,and curative effect is less affected by inflammatory state, and it has good short-term safety.

ABSTRACT

【Objective】 To investigate the expression of CD36 antigen in Suzhou area, analyze the type of antigen deficiency and gene mutation, so as to provide references for the establishment of CD36 negative donor registry in Suzhou. 【Methods】 Anticoagulant whole blood samples (805 cases) were randomly collected from healthy blood donors in Suzhou Blood Center. The expression of CD36 antigen on platelet and monocyte was analyzed by flow cytometry to determine the type of CD36 deficiency. The gene mutation type of platelet CD36 antigen-deficient was performed by genomic DNA sequencing. 【Results】 The CD36 deficiency frequency on platelet was 2.48% (20/805), among which TypeⅠ(lacking CD36 expression both on platelet and monocyte) and TypeⅡ(lacking CD36 expression on platelet only) CD36 deficiency accounted for 10% (2/20) and 90% (18/20), respectively. CD36 gene mutations were found in 10 samples, including 3 cases of 329_330 delAC, 1 case of 1228_1239 delATTGTGCCTATT and 2 cases of 1163 A>T; 1 case of 329_330 delAC+ 1172_1183 delTATTGGTCAAGC and 287 G>C+ 329_330 delAC heterozygous mutation. In addition, 1 case of 745 A>G and 1 case of 806 C>T mutations were novel, and not yet reported. 【Conclusion】 Results showed that the frequency of CD36 antigen deficiency in Suzhou were similar to that reported in southern China, but the mutation sites were slightly different. The establishment of CD36 negative platelet registry could provide negative platelets for patients with transfusion reactions caused by anti-CD36 antibody and improve the effect of clinical platelet transfusion.

ABSTRACT

Glaucoma is a chronic neurodegenerative disease of the optic nerve accompanied by apoptosis of the retinal ganglion cells, atrophy and depression of the optic nerve, and visual field loss. However, the mechanism of high intraocular pressure and glaucomatous optic neuropathy has not yet been elucidated.Autophagy is a lysosomal degradative process, which eliminates bulk cytoplasmic constituents. In recent years, a large number of studies have shown autophagy to be closely related to the occurrence and development of glaucoma. These studies examined the stress response of ocular tissues to high intraocular pressure, optic nerve protection, optic disc remodeling, immune regulation, abnormal protein removal and scar regulation. This article summarizes the relationship between autophagy and glaucoma, and pays special attention to the progress of research concerning autophagy in the trabecular meshwork, retinal ganglion cells, exfoliation syndrome, and scarring after glaucoma filtering surgery. It will lay a foundation for a deeper understanding of glaucoma pathogenesis and provide a new idea for diagnosing and treating glaucoma in the future.

ABSTRACT

Objective To identify an unknown pathogenic strain (X1) isolated from blood culture of an inpatient and to conduct a phylogenetic analysis on it. Methods The X1 strain was isolated from blood culture of an inpatient in the Second Xiangya Hospital of Central South University in 2016. It was identified by morphology biochemical testing and 16S rRNA gene sequencing. Based on genetic distances, a phylogenetic tree was constructed to compare the X1 strain with its homologous species in GenBank in order to reveal its genetic evolution. Results The X1 strain was gram-negative. Biochemical testing for oxidase, glucose and V-P reaction were positive (+) , while the result of citrate was negative (-) . Sequence align-ment analysis of 16S rRNA showed that the X1 strain (1466 bp) was highly homologous with Aeromonas veronii X71120. 1 with a similarity of more than 99%. Genetic distances between the X1 strain and its ho-mologous species, including Aeromonas veronii, Aeromonas hydrophila and Aeromonas sobria, were calculat-ed by MEGA5. 0 and the results revealed that the genetic distance (0. 014) between it and Aeromonas veronii X71120. 1 was the shortest, but the subspecies of it was unable to identify. Cluster analysis also indicated that the X1 strain and Aeromonas veronii were clustered to a small branch. Conclusions The X1 isolate was an Aeromonas veronii strain. Besides, Aeromonas veronii and Aeromonas sobria were similar in genetic char-acteristics.
